Everyone is talking about it, but few people really understand a person’s underlying barriers to better health. Please join us for a discussion about the difficulties of incorporating Social Determinants of Health (SDoH) in everyday practice, and how to overcome those challenges. This webinar will provide a framework for identifying and measuring SDoH to impact real world scenarios.
We will provide a view into Carrot Health’s Social Risk Grouper (SRG), a framework that leverages consumer data to identify and address SDoH risks effectively. SRG helps healthcare organizations measure the impact of SDoH risks on adverse health outcomes, such as co-morbidities, Emergency Department use, readmissions, and mortality. SRG may also be leveraged to measure outcomes of an intervention for longitudinal reporting.
In this webinar, you will learn:
· How to utilize a new SDoH taxonomy using consumer data to prioritize interventions and outreach
· How to augment risk stratification using SRG scores
· How to leverage consumer data to build an advanced risk identification & stratification methodology for early identification of rising risk among your covered population
· How to demonstrate ROI for SDoH interventions and programs

The traditional approach to member outreach for health plan quality improvement – using observed open gaps in care and open HCC recapture data gathered from clinical data sources – can improve quality metrics and outcomes. But these resources do not capture the unique consumer behaviors, interests, and aptitudes of each individual. This may lead to misalignment in timing, modality, and message – and ultimately unnecessary expense, member abrasion, and failure to spur members to close gaps.
Taking cues from our marketing and communications colleagues, we can harness consumer data and predictive analytics to drive highly effective tactical outreach.
Please join us for an in-depth and data-driven discussion on how you can apply modern marketing approaches, in concert with clinical data, to create diverse and intelligent member outreach campaigns that lead to improved quality outcomes.
In this webinar, you will learn:
- What consumers are saying about how they want to be engaged by their health plan
- Strategies for member outreach that maximize the utility of clinical data, drive intelligent resource allocation, reduce member abrasion, and improve overall member experience
- How to make campaign management a closed loop that maximizes feedback from your outreach investment and puts your member engagement in a perpetual state of learning

Social Determinants of Health (SDoH) live upstream from the healthcare system. When left unresolved, they can flow into the healthcare system as claims.
Social determinants can be addressed with the right tools. When managing value- or risk-based contracts, solving them can be the biggest lever for generating the triple aim: better outcomes and experiences while lowering costs in vulnerable populations.
Kurt Waltenbaugh, Founder & CEO of Carrot Health, and Jamo Rubin, MD, Founder & CEO of TAVHealth, will describe how to identify high social risk members within your managed population, and then how to leverage your community to help resolve their social determinants.
This presentation will cover:
- How data and predictive modeling can deliver hyper-local consumer segmentation based on underlying SDoH
- How to turn your town into a team by increasing collaboration among your community-based organizations to solve SDoH
- How a team approach generates outcomes that impact your bottom line while increasing the health and experience of your members
